The cardiac rehab program provides safe exercise training, one-on-one and group education, and counseling to improve your endurance, physical well-being, and quality of life. Our pharmacists have advanced training in cardiovascular medicine, and the management and treatment of heart problems. If you need an anticoagulant, or blood thinner, our pharmacists monitor you for health changes or problems, adjusting your medications as needed.
Nurse navigators and transitional care coordinators in our HeartCare Program ease your transition from the hospital back to home or to a care facility. Our registered dietitians have specialized training in heart disease, diabetes, and other conditions that affect heart health. You can meet with a dietitian to develop a dietary program that takes your specific health concerns into consideration, such as a low-sugar or low-sodium diet.
Our palliative care team provides care and support to patients and loved ones faced with a serious heart condition. We help manage pain, anxiety, depression, and other symptoms, as well as address emotional and spiritual needs, and coordinate care.
