AHN’s Healthcare@Home can provide a full range of quality care services right to your door, including home health care, palliative care, medical equipment and supplies, infusion therapy, and hospice care.
We also provide many of the outpatient services above online. We can conduct comprehensive geriatric assessments using telehealth services as well as consultations with geriatric primary care providers.
Jefferson Hospital’s Senior Plus Service links older adults to hundreds of programs and services available to South Hills, Steel Valley, and Mon Valley residents. We help older adults or their caregivers by matching their individual needs to programs and services. We do this with care, concern, and confidentiality.
